"Psoriasis is recognised as a chronic inflammatory immune-mediated multisystem disorder,1 affecting the skin, nails, and joints. This session provides a comprehensive overview of the epidemiology, types, and clinical manifestations of childhood psoriasis compared to adult onset, severe forms of the condition in children, associated comorbidities, differential diagnosis, treatment algorithms, challenges in paediatric psoriasis management, and the FDA and EMA approved medications, focusing on complex cases.
The eminent speaker highlighted the prevalence of childhood psoriasis in North India among children aged 6-14 years and in South India among those under 14 years old. The disorder exhibits a bimodal age of onset, distinguishing between type I and type II psoroiasis. Clinical characteristics of plaque psoriasis, anogenital psoriasis, and guttate psoriasis were discussed in children. The session featured severe variants of childhood psoriasis, including generalised pustular psoriasis of von Zumbusch, annular pustular psoriasis, psoriatic erythroderma, etc. while highlighting associated comorbid conditions, such as hyperlipidaemia, hypertension, diabetes, rheumatoid arthritis, and Crohn’s disease. Early detection and timely treatment initiation were underscored, alongside differential diagnoses like acrodermatitis enteropathica, candidiasis, eczema, pityriasis rosea, and so on. The session emphasised considerations for initiating therapy in children, including topical treatments with emollients, moisturizers, keratolytic agents, corticosteroids, vitamin D analogues, calcineurin inhibitors, anthralin, and retinoids.2 Phototherapy and systemic therapies such as Methotrexate, Cyclosporine, and Biologics were also discussed, addressing challenges encountered during treatment. The FDA and EMA approved certain medications, such as topical corticosteroids, anthralin, Adalimumab for childhood psoriasis, which were also reviewed. Various case scenarios were analysed to enhance clinical knowledge and practical application in managing childhood psoriasis effectively.
